2nd Step
Upon receipt of your inquiry, we will provide you with a screening questionnaire. Your responses will help us ensure that both psilocybin and this group experience are aligned with your current needs and circumstances. Please return this questionnaire no later than April 20, 2024.
3rd Step
As soon as we’ve received your completed questionnaire, we will schedule your discovery call. During that call we’ll confirm your official seat in the group. Then you’ll complete your Oregon Health Authority (OHA) intake forms in advance of your first preparation session.

60-min 1:1 intake

Psilocybin fee of $5.50/mg + 15% Oregon State Tax

Group Integration Session 2: May 29th at 1pm PT; 4pm ET (via Telehealth)
